Notes on completion: If you are accessing this form through WEBFLEET, some details should be auto-completed, *please do not amend the account code. The Name field should be completed as Firstname Surname (eg Bob Smith - with a space between first and last names). The UniqueID is whatever identification code you use to identify drivers internally. Tacho numbers and PINs are optional fields. The PIN is the last four digits of your driver's mobile number, which will become the PIN to access the tablet (please note, first digit cannot be 0). Please select the radio button for 'amend' if you are updating details for an existing driver (new is the default selection).
